- On March 24, the Ministry of Education and Training announced the registration form for the 2016 National High School Exam. Below is the registration form and instructions on how to fill in 19 items in the registration form as instructed by the Ministry of Education and Training.

Candidates should note that before filling out the exam registration form (DKDT), candidates must carefully read the items and detailed instructions on the back of Form No. 2. If there is anything unclear, candidates must ask the DKDT reception officer for full instructions.

Candidates must fill in all the items on the front of the Registration Form, Form No. 1 and Form No. 2, then submit them to the registration reception office along with a photocopy of both sides of the Identity Card or Citizen Identification Card on one side of an A4 sheet of paper and 2 4x6 portrait photos, taken within the last 6 months (with full name, date of birth, province, district and registration unit code clearly written on the back of the photo, these 2 photos are contained in a small envelope).

In addition, a photo must be attached to the designated position on the front of the exam registration envelope. The high school where the candidate is studying or the local police station where the candidate is residing must sign and stamp the photo (only to confirm the candidate's identity). The candidate is fully responsible for the information declared in the exam registration form.

The place receiving the registration will keep the bag containing the registration form, form number 1, a photocopy of the ID card or citizen identification card and 2 photos, and return form number 2 to the candidate after signing and stamping the confirmation.

Candidates keep this Form No. 2 to receive the Exam Notice and Certificate of Exam Results. In case of any errors in the exam registration information or loss of the Exam Notice, candidates must bring this Form directly to the Examination Council at the registered Examination Cluster at the meeting to disseminate the regulations to request correction of errors and complete the exam procedures.

How to fill in 19 items in the 2016 National High School Exam Registration Form as follows:

Section DEPARTMENT OF EDUCATION AND TRAINING……… DEPARTMENT CODE: Candidates registering at the exam registration unit belonging to which department, write the name of that department in the blank space …..., then fill in the 2 digits representing the department code in the next 2 blank boxes, the Department of Education and Training code is prescribed by the Ministry of Education and Training.

Registration Number: The place where the exam registration is received must fill in this section. Candidates must not fill in this section.

Section 1, 2: Fill in according to the instructions on the registration form.

Section 3: a) The candidate's place of birth only needs to clearly state the name of the province or city. If born abroad, the candidate only needs to clearly state the name of the country (in Vietnamese). b) Ethnicity must be recorded exactly as on the birth certificate.

Section 4: For old-model ID cards, write 9 digits in the last 9 boxes on the right, leave the first three boxes blank; for new-model ID cards or Citizen Identification Cards, write all 12 digits in the corresponding boxes.

Section 5: The province (city) code, district (county) code and commune (ward) code for communes in Region 1 will be determined by the Ministry of Education and Training only. Candidates need to look up at the place of registration to correctly enter the province (city) code, district (county) code, commune (ward) code where the candidate has a current permanent residence in the corresponding boxes on the right. Candidates who do not have a permanent residence in a commune in Region 1 should leave the commune code box blank. After filling in all administrative unit codes, candidates should clearly write the name of the province (city), district (county), commune (ward) in the blank line. For candidates who are prioritized for subjects or areas related to permanent residence, it is required to confirm that they have had a permanent residence for more than 18 months in Region 1 or more than 18 months in a commune with special difficulties during their high school years by checking the corresponding box.

Section 6: Write the school name and address to the district (county), province (city) of the school on the dotted line. Write the province code where the school is located in the first 2 boxes, write the school code in the next 3 boxes (school code is written according to the regulations of the Department of Education and Training, if the school code has 1 digit, write the number 0 in the first 2 boxes, if the school code has 2 digits, write the number 0 in the first box). For candidates who are police officers or soldiers sent to take the exam for university or college admission, write the province code corresponding to the province where they are stationed and the high school code is 900. For candidates who have studied abroad, the years of study abroad write the province code corresponding to the province according to their permanent residence in Vietnam and the high school code is 800.

Section 7: Clearly state your phone number and email (if any). For candidates who wish to apply online, they must register their personal mobile phone number to receive a one-time password (OTP) via text message to ensure security when applying online.

Section 8: Candidates must clearly state the full name of the contact person and detailed address: hamlet (house number), village (street, alley), commune (ward), district (county), province (city).

Section 9: Candidates register to take the exam with the purpose of only obtaining results for high school graduation, only obtaining results for university or college admission, or both purposes. Candidates register to take the exam with which purpose, mark "X" in the corresponding box; if candidates register to take the exam with both purposes, mark both boxes.

Section 10: Candidates registering for the exam at which cluster must write the cluster name and cluster code as prescribed by the Ministry of Education and Training in the corresponding position. Depending on the purpose of the exam stated in Section 9, candidates should refer to the instructions of the place receiving the exam registration to determine the appropriate cluster.

Section 11: Students studying in grade 12 of high school must submit their application at that school. Other subjects must submit their application at locations specified by the Department of Education and Training. The application unit code is recorded according to the instructions of the place receiving the application.

Section 12: All candidates registering for the exam for high school graduation recognition and university and college admission must register for the exam subjects in this section. Candidates registering for any exam subject must mark "X" in the corresponding exam subject box. For the Foreign Language subject, candidates must fill in the code corresponding to the specific language as follows:
N1 – English; N2 – Russian; N3 – French; N4 – Chinese; N5 – German; N6 – Japanese.

Section 13: For candidates requesting exemption from the foreign language exam, it is necessary to clearly state the type of certificate that qualifies for exemption or clearly state that they are a member of the national team participating in the International Olympic competition in Foreign Languages according to the regulations of the Ministry of Education and Training.

Section 14: Candidates who have taken the high school exam in previous years, if there are subjects that are eligible for retention according to regulations, candidates who want to retain a subject, write the score of that subject in the corresponding box.

Section 15: Candidates determine the 4 subjects used for high school graduation (including compulsory subjects and elective subjects) by marking “X” in the corresponding boxes. These four subjects must be among the subjects registered in Sections 12, 13 and 14.

Section 16: Candidates self-identify priority subjects, correctly write the priority subjects symbols according to the regulations of the regular university and college admissions regulations. If the declaration is not truthful, they will be handled according to current regulations. Priority candidates must submit all legal documents to the school upon admission.

Section 17: For candidates taking the exam for the purpose of university and college admission, it is necessary to write the region code in the blank box as follows: Region 1 (KV1) fill in the number 1, Rural Region 2 (KV2-NT) fill in 2NT, Region 2 (KV2) fill in the number 2, Region 3 (KV3) fill in the number 3. In the 3 years of high school or equivalent, the school where you studied longer will enjoy regional priority. If each year of studying at a school has a different regional priority level or half of the time studying at this school, half of the time studying at another school, then the high school graduation will enjoy regional priority in that area. For candidates who are given priority based on permanent residence, based on the regulations of the admission regulations and instructions of the Ministry of Education and Training, write the correct priority area. The exam registration software will automatically determine the regional priority level according to the Regulations on regular university and college admission when candidates fully fill in Sections 5 and 6; If the information is different from the information declared on the registration form, candidates need to check the information declared in these sections.

Section 18: Fill in according to the instructions on the registration form.

Section 19: For candidates taking the exam with the purpose of obtaining results to consider transferring to college or university, mark "X" in the box of the corresponding level of education graduated: Graduated from secondary school (TC) or Graduated from college (CĐ)

Note: Candidates must fill in all required fields completely, clearly, and neatly without correction or erasure. If it is a number, write it in Arabic numerals (0, 1, 2, 3, ...), not in Roman numerals (I, V, X, ...).
