How to fill in the application form as follows:
1. Registration number, ID number: Must be entered accurately and consistently with the information in the exam registration form.
2. Registration code: Write the exact registration code specified in the candidate's Examination Results Certificate.
3. "Request school to adjust priority regime" section: If candidates need to adjust priority regime (compared to information in the Registration Form), mark "X" in the corresponding box and in the "priority area" section, fill in one of the codes KV1, KV2-NT, KV2 or KV3 corresponding to their priority area; in the "priority object" section, fill in one of the symbols from 01 to 07 corresponding to their priority object (if not in the priority object, do not fill in).
4. "Priority admission" section: Candidates who do not use the right to direct admission and meet the requirements specified in Clause 3, Article 7 of the Admission Regulations, when registering for admission to the school, must mark "X" in the box next to it and at the same time:
a) Fill in after the "Object" item one of the symbols from 01 to 04 according to the following regulations:
- Candidates winning prizes in the national excellent student selection exam: 01;
- Candidates winning prizes in the National Science and Technology Competition: 02;
- Candidates winning gold medals in national first-class championships held once a year: 03;
- Candidates recognized as national masters by the Sports and Physical Training Committee: 04.
b) Fill in after the "Type of Award, Medal" section: the award, medal you have achieved;
c) Fill in after the "Prize-winning subject" section: the subject or sport in which the prize or medal was won. For candidates who won a prize in a National Science and Technology Competition, briefly write the name of the prize-winning topic.
5. Candidates confirm the priority by region and priority by admission subject. If the priority is different from the application, the box "Priority has changed compared to the application for the national high school exam " must be checked. Candidates are responsible for the accuracy of this information.
6. "Registration wishes" section:
Candidates do not have to register all their wishes. Unregistered wishes must be crossed out to confirm.
7. Section "Registered for admission to another school"
- Candidates only apply to 1 school or 1 group of schools: do not fill in information in this section;
- Candidates applying to 2 schools or 1 school and 1 group of schools: mark "X" in the box next to it and fill in all the information about the second school that the candidate applies to.
