The 2021 National High School Exam registration form was issued by the Ministry of Education and Training in Official Dispatch 343/QLCL-QLT dated March 25, 2021.

The registration period for the high school graduation exam and university entrance exam in 2021 is from April 27 to 5:00 p.m. on May 11. Candidates can register their admission wishes online from April 27 to 5:00 p.m. on May 16. To avoid unnecessary mistakes in the process of registering for the exam, Dai Nam University has compiled detailed instructions. Please follow the article below.

Instructions for filling out the 2021 National High School Exam registration form

Section DEPARTMENT OF EDUCATION AND TRAINING....... DEPARTMENT CODE: Candidates registering at the examination registration unit belonging to which department, write the name of that department in the blank space, then fill in the 2 digits representing the department code in the next 2 blank boxes, the Department of Education and Training code is prescribed by the Ministry of Education and Training.

Registration Number: The place where the exam registration is received must fill in this section. Candidates must not fill in this section.

Section 1, 2: Fill in according to the instructions on the Registration Form for the High School Graduation Exam and Admission to College and University in Early Childhood Education (hereinafter referred to as the Registration Form).

Section 3: a) The candidate's place of birth only needs to clearly state the name of the province or city. If born abroad, the candidate only needs to clearly state the name of the country (in Vietnamese). b) Ethnicity is recorded exactly as on the birth certificate. c) For foreign nationality, mark (X) in the box next to it.

Section 4: For old-model ID cards, write 9 digits in the last 9 boxes on the right, leave the first three boxes blank; for new-model ID cards or Citizen Identification Cards, write all 12 digits in the corresponding boxes.

Section 5: The province (city) code, district (county) code and commune (ward) code for communes (wards) in Region 1 will be determined by the Ministry of Education and Training only. Candidates need to look up at the place of registration to correctly enter the province (city) code, district (county) code, commune (ward) code where the candidate has current permanent residence in the corresponding boxes on the right.

Candidates who do not have a permanent residence in a commune in Area 1 should leave the commune code box blank. After filling in all administrative unit codes in the boxes, candidates should clearly write the name of the commune (ward), district (county), province (city) in the blank line.

For candidates who are prioritized in subjects or areas related to permanent residence, it is required to confirm that they have had permanent residence for more than 18 months in area 1 or more than 18 months in a particularly disadvantaged commune during high school by checking the corresponding box.

Section 6: Write the school name and address to the district (county), province (city) of the school on the dotted line. Write the province code where the school is located in the first 2 boxes, write the school code in the next 3 boxes (school code is written according to the regulations of the Department of Education and Training, if the school code has 1 digit, write the first 2 boxes with the number 0, if the school code has 2 digits, write the first box with the number 0).

For candidates who are police officers or soldiers sent to take the exam for university and college admission, write the province code corresponding to the province where they are stationed and the high school code is 900.

For candidates who have studied abroad, the years of study abroad must be recorded as the province code corresponding to the province according to the permanent residence registration in Vietnam and the high school code is 800. Class name section: clearly state the name of the 12th grade where the student is studying (for example 12A1, 12A2,...), for students who are independent candidates, write "TDO".

Section 7: Clearly state phone number and email. For candidates who request to adjust their online admission registration, they need to register their personal mobile phone number to receive a one-time password (OTP) via text message to ensure security when registering online.

Section 8: Candidates must clearly state the contact information: full name; phone number; hamlet address (house number), village (street, alley), commune (ward), district (county), province (city). This address is also the address to receive the Admission Notice if the candidate is admitted.

Section 9: Candidates who wish to use the exam results to consider admission to university or college in Early Childhood Education, please mark (X) in the box next to it.

Section 10: Candidates must mark (X) in one of the two boxes to clearly indicate whether they are studying under the high school program or the continuing education program.

Section 11: For independent candidates, mark (X) in one of the two boxes to clearly distinguish whether the independent candidate has not graduated from high school or has graduated from high school (at the time of the exam).

Section 12: Candidates registering to take the exam at which examination council must write the name of the examination council and the examination council code prescribed by the Ministry of Education and Training in the corresponding position.

Section 13: Students studying in grade 12 of high school must submit their registration at that school. Other subjects must submit their registration at locations specified by the Department of Education and Training. The registration unit code is recorded according to the instructions of the place receiving the registration.

Section 14: For candidates who are currently 12th grade students (not yet graduated from high school), they must register for the exam at point a, candidates are not allowed to choose the component subjects at point b. For independent candidates, depending on the purpose of the exam, depending on the choice of subject combinations for university and college admission, they can choose the entire exam (at point a) or only choose some component subjects (at point b) as appropriate.

Candidates can only register for one combined exam (Natural Science or Social Science). Independent candidates can only register for a component exam in the same combined exam. In case independent candidates have not graduated from high school and have exams/subjects (for graduation recognition) from the previous year that are eligible for retention, if they want to retain any exam/subject, they must record the score of that exam/subject in Section 16.

However, candidates can still choose to take the component exam/subject (which they have reserved) to get the results for university and college admission. Candidates studying under the GDTX program can choose the foreign language exam if they wish to use a foreign language in the subject group for university and college admission.

How to choose a test/component subject: Candidates who register for a test/component subject should mark (X) in the corresponding test/component subject box. For the Foreign Language test, candidates should fill in the code corresponding to the specific language as follows: N1 - English; N2 - Russian; N3 - French; N4 - Chinese; N5 - German; N6 - Japanese. Candidates can only choose the component subjects in a combined test.

Section 15: For candidates who wish to be exempted from the foreign language exam or register for admission, it is necessary to clearly state the type of certificate that qualifies for exemption or clearly state that they are a member of the national team participating in the International Olympic in Foreign Languages according to the regulations of the Ministry of Education and Training or candidates must state the type of foreign language certificate to register for admission according to the requirements of the University or College of Education. For the type of certificate that has the exam score (total exam score), candidates must write the score in the "Exam score" box.

15. Register for exemption from foreign language exam or register for admission consideration: (Candidates write the type of foreign language certificate that meets the requirements according to regulations or clearly state that they are a member of the national team participating in the International Olympic in Foreign Languages to be exempted from the exam or candidates write the type of foreign language certificate to register for admission consideration according to the requirements of the University or College of Education): TOEFL ITP

Section 16: Candidates who have taken the high school exam last year, if there are any exams/subjects that are eligible to be reserved according to regulations, candidates who want to reserve the scores of any exam/subject must write the scores of that exam/subject in the corresponding box (Note: For combined exams, candidates are entitled to reserve the results of each component exam if eligible, to reserve the entire score of any combined exam, candidates must write the scores of all component exams of that combined exam).

For reserved exams/subjects, candidates can still register to take the exam (exam or component subject) in Section 14 only if they wish to use the exam results for university and college entrance examinations.

Section 17: Candidates self-identify priority subjects, correctly write the symbols of priority subjects according to the regulations on university level admission; college level admission in Early Childhood Education and guidance documents. If the declaration is not truthful, it will be handled according to current regulations. Priority candidates must submit all legal documents and evidence to the school upon admission.

Section 18: For candidates taking the exam for the purpose of university and college admission, write the region code in the blank box as follows: Region 1 (KV1) fill in the number 1, Rural Region 2 (KV2-NT) fill in 2NT, Region 2 (KV2) fill in the number 2, Region 3 (KV3) fill in the number 3.

In the 3 years of high school or equivalent, the place where you study longer will enjoy regional priority. If you study at a school with a different regional priority each year or study half the time at one school and half the time at another school, then the place where you graduate from high school will enjoy regional priority there.

Section 19, 20: Fill in according to the instructions on the registration form.

Section 21: Fill in according to the instructions on the registration form.

For point a:

  • Information on online admission application registration has the same legal value as registration by form.
  • Candidates are responsible for the information they have declared in the online application system.
  • The wishes are arranged in order of priority from top to bottom (wish 1 is the highest priority wish).
  • Candidates who register any wishes that do not comply with the regulations of the schools will not be entered into the software system for the school to consider.

Note:

Independent candidates are: those who have completed high school but have not yet taken the high school graduation exam or have taken the exam but have not yet graduated from high school in previous years; those who have a high school graduation certificate or a secondary school graduation certificate taking the exam to use the results as a basis for admission registration.

Candidates must fill in all required fields completely, clearly and neatly without correction or erasure.

If it is a number, write it in Arabic numerals (0, 1, 2, 3,...), do not write it in Roman numerals (I, V, X,...).

Candidates use the account and password provided after submitting the application to log in to the QLT system via the internet at http://thisinh.thitotnghiepthpt.edu.vn to register their admission wishes online.
