Ministry of Education and Training announces registration form for 2021 high school graduation exam

Accordingly, before taking the 2021 high school graduation exam, candidates must submit an application for the exam according to the unified form prescribed by the Ministry of Education and Training (MOET).

High school graduation registration documents for 2021 for students studying in grade 12 , including: 2 identical registration forms; high school transcript (original or copy); high school continuing education transcript or test form of self-study learners for continuing education issued by the principal of the high school; household registration book (copy) to enjoy preferential treatment related to the place of permanent residence registration; 2 photos 4x6cm in size; valid certificates to enjoy preferential treatment if any.

When submitting the high school graduation registration form, candidates must use the correct registration form as prescribed by the Ministry of Education and Training and fill in all information according to the new form.

Independent candidates who have graduated from high school and want to retake the exam for university or college admission need to prepare the application documents including: 2 identical application forms and must use the correct form according to regulations; copy of high school diploma; 2 4×6cm photos; 2 envelopes with stamps and clearly stating the candidate's contact address; notarized or certified identity card or citizen identification card; household registration book (copy); certificate of preferential treatment if any.

This year's registration form has a notable new feature:

+ For Section 21 of the Admission Registration and Examination Form (ADC), candidates should carefully study the admission information of the university or college they wish to study in order to have accurate information about the school code, major/major group code, major/major group name and subject combination code for admission.

Fill in according to the instructions on the registration form. For point a: information about online registration of admission wishes has the same legal value as registration of admission wishes by form. Candidates must take responsibility for the information about online registration of admission wishes declared on the system.

The wishes are arranged in order of priority from top to bottom (wish 1 is the highest priority wish). If a candidate registers a wish that does not comply with the school's regulations, that wish will not be entered into the software system for the school to consider.

+ Candidates who are grade 12 students: submit the application directly to the high school where the candidate is studying. Independent candidates: after preparing the application, independent candidates will submit the application for high school graduation at the district/county Department of Education and Training.

Candidates must fill in all required fields completely, clearly and neatly without correction or erasure.

- If it is a number, write it in Arabic numerals (0, 1, 2, 3...), do not write it in Roman numerals (I, V, X...).

- Candidates use the account and password provided after submitting the exam registration form to log in to the exam management system via the Internet at http://thisinh.thitotnghiepthpt.edu.vn to register for admission online.

The 2021 high school graduation exam is scheduled to take place from July 6 to July 9, of which July 7 and 8 are the official exam dates.
