NOTIFICATION

Regarding registration to purchase additional health insurance for students for the 2015 - 2016 school year

To: Faculties, Departments, Institutes, Centers;

Homeroom teachers;

Pursuant to Decree No. 105/2014/ND-CP dated November 15, 2014 of the Government detailing and guiding the implementation of a number of articles of the Law on Health Insurance;

Pursuant to Joint Circular No. 41/2014/TTLT-BYT-BTC dated November 24, 2014 of the Ministry of Health - Ministry of Finance guiding the implementation of health insurance;

Pursuant to Official Dispatch No. 1656/BHXH-PT dated August 6, 2015 of Hanoi Social Insurance regarding guidance on supplementing health insurance for students;

The 2015-2016 school year is a transition year from participating in student health insurance by academic year to participating by fiscal year, so the period for participating in health insurance for Dai Nam students is divided into many different periods. The school has implemented the purchase of student health insurance for the entire school, however, up to now, there are still students who spontaneously pay. To unify the management and implementation of health insurance collection for Dai Nam students from January 2016 to the end of December 2016, the school has announced the following additional registration for purchase:

1. Health insurance contribution level = 1,150,000 VND (Basic salary) x 4.5% x number of months of participation x 70%.

Monthly contribution = 36,225 VND/month

2. Participants and card validity period:

2.1. For students who register to purchase from January 2016: Number of months required to participate: 12 months and the card is valid from January 1, 2016 to December 31, 2016.

2.2 For students who purchased health insurance in 2015 and are valid in 2016: The number of months required to participate is calculated from the month after the health insurance card expires until December 31, 2016.

For example: Students buy health insurance valid from October 1, 2015 to August 30, 2016. The next purchase period is from September 1, 2016 to December 31, 2016.

2.3 For final year students: The number of months required to participate is calculated from the time of registration to the last day of the month ending the school year (June 30, 2016).

3. Time and place of collection and payment:

Time : Students pay during office hours.

Phase 1: from December 25, 2015 to January 5, 2016 (for students purchasing health insurance under categories 2.1, 2.3).

Phase 2: from March 25, 2016 to April 5, 2016 (for students purchasing health insurance under categories 2.2, 2.3).

We request that the Heads of Faculties, Departments and homeroom teachers inform all students to know and implement.

Attention:

- Students must pay on time as prescribed. After this time, the school will stop collecting student health insurance for the 2015-2016 school year.

- From the 2016-2017 school year onwards, students participate in paying health insurance according to the fiscal year (from January 1 to December 31 every year) according to the provisions of Joint Circular No. 41/2014/TTLT-BYT-BTC;

- When there is a notice of change in the basic salary, the school will collect health insurance premiums in accordance with regulations from the effective date.
